Heartwatch - Community Cardiac Rehabilitation Scheme

Heartwatch is a community based exercise programme for cardiac rehabilitation and secondary prevention of heart disease. It is aimed at men and women of any age who have suffered heart problems, including:

  •  Heart disease,
  •  Angina,
  •  Heart attacks
  •  Bypass surgery
  •  Valve replacement
  •  Transplant surgery

Anyone with cardiac disease is eligible, providing we can gain a referral from your GP, Cardiac Nurse or Consultant.

Heartwatch is overseen by specialist instructors who are qualified in cardiac rehabilitation. Our team will support you through your initial assessments and through your twelve week programme. There is also an opportunity to stay in contact with us after your twelve weeks if appropriate.

Everyone is different, so your cardiac rehabilitation programme is tailored to your needs. You will be advised on a suitable programme based on your medical history and assessment results.

You will be able to access our specialist Heartwatch circuit classes or may be able to attend a gym session under the guidance of our instructors. You may also be able to access some of our Fitness For Life sessions if that is more appropriate for you.

How do I benefit?

Exercise is a very important part of your rehabilitation after a heart attack or surgery, as it will help your recovery and improve your fitness, strength and general well-being. Regular exercise also helps to keep you healthy in the future and helps prevent and further heart problems.

Cardiac rehabilitation also helps to improve your confidence after an event and return to your usual activities more quickly. You will be supported and learn how to 'pace yourself' to exercise safely.

Joining one of our group sessions will also give you the chance to meet other people who have been through the same thing, you can ask questions and talk about any worries you may have. It can be good fun too!

The next step...

  • If you have had a heart attack or surgery, your cardiac nurse will speak to you about your rehabilitation programme. You will be offered community cardiac rehabilitation at an appropriate time providing your condition is stable. Your referral will be sent through to the Healthy Lifestyle Team and we will contact you for an initial appointment.

  • If you have other heart problems such as angina or a pacemaker you can be referred by your GP or Practice Nurse. In this case they will give you a copy of the referral form, you will find our contact details at the bottom of the form. Contact the Healthy Lifestyle Team for your initial appointment and assessment.
